    Having begun her career within the pharmaceutical field, Danella developed a strong desire to pursue a healthcare vocation in which she could enjoy increased personal interaction with patients. This is what led her to the field of nursing. She worked as a cardiovascular intensive care nurse at Medical City Fort Worth where she received many accolades including both the Daisy Award and Fort Worth Business Presses’ Healthcare Hero. Still, she recognized that the ability to build and keep lasting one-on-one relationships was missing. Realizing her passion for patient care went beyond bedside nursing, she pursued an advanced practice degree at the University of South Alabama. As both a family and acute care nurse practitioner, Danella believes in the melding of holistic and conventional interventions to improve patient health and well-being.
    Born in Norman, Oklahoma, Danella moved to the DFW metroplex as a teenager. She now lives in Alvarado, TX with her husband and two children. When she is not relaxing with her cats, she spends her off-time traveling and creating new memories with her family.

    Diana Dang, APRN, NP-C

    Fort Worth Primary Care

    Diana, APRN, NP-C is a bilingual (English & Vietnamese) board-certified nurse practitioner. She was born in Omaha, Nebraska but grew up in Waco, Texas. She graduated with a Bachelor of Science in Nursing at Baylor University. She then spent 10 years working as a Registered Nurse in a demanding cardiothoracic unit and surgical recovery unit at Baylor University Medical Center of Dallas and then Medical City of Fort Worth. After acquiring invaluable skills in those settings, Diana decided she wanted to provide an even higher level of care for patients. Furthermore, she went on an overseas mission trip which deepened her desire to serve people who are often overlooked. These experiences led Diana to enroll at Spring Arbor University and attain her Nurse Practitioner degree.
    Now, she believes her calling is to serve as a nurse practitioner to improve the quality of life for the hurting and those in need. She believes in treating her patients with dignity and compassion.
    In her spare time, Diana enjoys traveling, listening to music, baking, arts/crafts, doing mission work, and spending time with family and friends.

    Jose J. Lozano, DO, MS, FACOI

    Burleson Primary Care Physician

    Dr. Lozano was born and raised in El Paso, Texas. He moved to North Texas to pursue his lifetime dream of becoming a physician. Dr. Lozano obtained his medical degree from the University of North Texas Health Science Center – Texas College of Osteopathic Medicine and completed his internal medicine residency at Plaza Medical Center of Fort Worth where he was nominated to serve as the clinic chief during his residency.
    Dr. Lozano believes in empowering his patients with knowledge of their own health, body and mind. As a former biology adjunct assistant professor in El Paso, he enjoys explaining and teaching disease processes, medication interactions and overall good health habits so that he and his patients can work together as a team.
    In his off time, Dr. Lozano enjoys traveling, cooking, and spending time with friends and family.

    Omesh Singh, DO, FACOI

    Burleson Primary Care Physician

    Dr. Singh was born and raised in Arlington, Texas where he realized from an early age that he wanted to be a physician. This desire took him to the University of Texas at Austin where he completed his Bachelor of Arts in Biology and became an emergency medical technician during his transition to medical school.
    Dr. Singh attended the Lake Erie College of Osteopathic Medicine in Florida where he realized that his true calling was in internal medicine. He enjoyed the challenges and diversity he met through connecting with patients from all walks of life. He quickly discovered his bedside manner and his ability to tap into the potentials of his patients, helping them to reinvigorate better health and better outlooks despite their ailments.
    This Texan realized that he needed to come back home to Fort Worth to further his training, and he successfully completed his residency in internal medicine at Plaza Medical Center of Fort Worth. During residency, Dr. Singh enjoyed educating his patients about their various health conditions and was repeatedly recognized for his unique humanistic approach to medicine. He continuously brings ease to his patients and their families.
    Dr. Singh has been identified as innovative in his approach to medicine where he incorporates a holistic avenue of how diet, exercise and daily activities can affect disease processes. He has particular interests in hypertension, diabetes, preventative medicine, and weight loss.

    A Passion for Helping Others

    He is very passionate about community service and participates in an annual medical mission trip. His most recent was a visit to the southern coast of Jamaica, where he and his team of 14 medical students and nurses cared for the Jamaica’s indigent.
    Dr. Singh enjoys a balanced life with exercise, traveling, cooking various cuisines, music, religion, sports, and spending time with his loved ones.

I have a commitment to my patients and their families to become their guide and support as they achieve their optimum health while facing various ailments.     Raju Raghunath, MD

    Dr. Raghunath completed his medical education at Thanjavur Medical college, Dr. M G R Medical University, India. After moving to United States, Dr. Raghunath served his residency in Family Medicine at Plaza Medical Center/UNTHSC, Fort Worth.

    Based on his professional experience in the United States and India, he thought primary care is a challenging specialty where he gets to treat a variety of interesting and demanding cases. He has dedicated his professional life to the care and treatment of patients of all ages, creating long lasting relationships with patients and their families.

    Dr. Raghunath enjoys traveling, grilling and watching football with his adorable kids.

    Trey Tullous, MSN, APRN, FNP-C

    Family Nurse Practitioner Trey is a 5th generation Fort Worth, Texan and a board certified Family Nurse Practitioner. While living in New York City, he realized his calling to the nursing field, earned his Associates of Nursing in 2009 and started his career in the emergency department at Jersey City Medical Center. In 2012, Trey and his family moved back home to Texas where he continued nursing in the emergency department at THR Harris Hospital, while earning his BSN from Excelsior College and starting his MSN at The University of Texas at Arlington in 2016.
    In 2020, Trey joined Baylor Scott and White All Saints Hospital where he served on a Med/Tele floor as a Charge Nurse and front-line nurse, caring for inpatient Covid positive patients. He graduated from UTA with his MSN-FNP in 2020.
    Over the years, Trey has developed a passion for healthy living, patient education and patient-centered care. His hope is that his compassion and love of medicine and people is evident in the care he provides his patients.
    Trey loves spending time with his amazing wife of 13 years and their two boys. He loves being outdoors, exercise and physical fitness, new restaurants, music and singing, and is an active member of his church choir
